Frequently Asked Questions

Everything you need to know before your first visit.

At Catalyst, we put patients first. Every program is customized to your goals, not a one-size-fits-all protocol. Each session is a full hour, one-on-one with a Doctor of Physical Therapy, with no distractions and no aides splitting your care.

Our goal isn't just to get you out of pain. It's to get you back to doing what you love, feeling better than you did before. Unlike traditional PT models, our clinical decisions are driven by what's best for you, not by what insurance will allow.

At Catalyst, we believe your insurance company should not dictate when you can and cannot receive care based on what is "medically necessary." We are out-of-network with health insurance, which allows us to get you better results in fewer visits. Your therapist will determine how many sessions you need to achieve your goals.

Payment is due at the time of service, but we will provide you with a superbill that you can submit to your health insurance for out-of-network reimbursement. You can also use your FSA or HSA.

The investment for a one-time evaluation is $230. Your Doctor of Physical Therapy will recommend a treatment package tailored to you, with sessions averaging between $190 and $200 each.

Your first visit is a full initial evaluation. You'll spend the entire time one-on-one with your physical therapist, no handoffs or shared attention. Together, you'll identify the root cause of your pain or injury and build a treatment plan designed to get you the results you're looking for.

Wear something you can move comfortably in, like workout clothes. Please bring your preferred method of payment and fill out your new patient forms in the patient portal before your appointment!

Illinois is a direct access state, meaning you do not need a referral or script from your doctor to start Physical Therapy. Some insurances may still require a script for reimbursement, so check with your provider to be sure!

Patient Rights

Your Right to a Good Faith Estimate

Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
